I had the privilege of attending an international investigator meeting in Tokyo, Japan, as a Principal Investigator for a clinical study on hidradenitis suppurativa (HS). This chronic inflammatory skin disease can cause considerable pain and have a profound impact on patients’ daily lives.

It is an honour to contribute to research that seeks to expand our understanding of HS and explore potential new treatment options. The meeting brought together investigators and research professionals from different countries to review the study protocol, discuss patient safety, and coordinate the practical aspects of conducting an international multicentre clinical trial.

Alongside the scientific discussions, the visit offered a welcome opportunity to reconnect with international colleagues and enjoy the warm hospitality of friends in Japan.

Understanding Hidradenitis Suppurativa

Hidradenitis suppurativa is a chronic inflammatory skin condition characterised by recurrent painful nodules, abscesses, draining tunnels, and scarring. It commonly affects the armpits, groin, buttocks, and areas beneath the breasts.

HS is often under-recognised, and diagnosis may be delayed. Some patients experience years of recurrent painful swellings and drainage before their condition is correctly identified.

Its effects extend beyond the skin. Pain, discharge, and recurrent inflammation can interfere with sleep, mobility, work, and everyday activities, while also affecting self-confidence and emotional well-being.

Early recognition and appropriate management are important to control inflammation, limit further tissue damage, and improve quality of life.

Why Clinical Research Matters

Managing hidradenitis suppurativa can be challenging, particularly in patients with moderate to severe disease. Depending on the severity and individual needs, treatment may involve topical or systemic medications, biologic therapies, surgical procedures, and ongoing care to manage symptoms and flare-ups.

Despite advances in treatment, some patients continue to experience persistent disease. Clinical research remains essential to address these unmet needs and evaluate potential new approaches.

Clinical trials systematically assess the safety and effectiveness of investigational treatments. The evidence they generate helps determine whether these treatments may have a role in future clinical practice.

As a Principal Investigator, my responsibilities include overseeing the study’s clinical conduct at the participating research site, prioritising patient safety, adhering to the approved protocol and ethical requirements, and ensuring the quality of clinical data.

This role is both a professional responsibility and a meaningful opportunity to contribute to the evidence that shapes future patient care.

Learning and Collaboration in Tokyo

The investigator meeting provided a valuable setting to review the study objectives and protocol, discuss research methodology, and align approaches to clinical assessment and safety monitoring across participating centres.

International multicentre trials depend on close collaboration between investigators, research teams, and study organisers. Consistent assessments and careful adherence to study procedures are essential to producing reliable, meaningful results.

Meeting fellow investigators also offered an opportunity to exchange perspectives and learn from colleagues working in different clinical settings. These conversations reinforced the value of international collaboration in addressing the challenges of chronic inflammatory skin diseases.

Reconnecting with International Colleagues

A particularly memorable part of the visit was the opportunity to catch up with Professor Koremasa Hayama from the Department of Dermatology, Nihon University, Japan, as well as Dr Rosa from Spain.

I was grateful to Professor Hayama for his warm hospitality and for treating me to a wonderful Japanese dinner. Sharing local cuisine and catching up in a relaxed setting made the evening especially enjoyable.

These personal connections are among the most rewarding aspects of international academic meetings. Relationships formed through dermatology can grow into lasting friendships, with opportunities to share experiences and support one another beyond the formal scientific programme.

Bringing Research Perspectives into Clinical Practice

Participating in clinical research deepens my appreciation of how potential treatments are developed and evaluated. It also reinforces principles that remain central to everyday dermatology practice: careful clinical assessment, patient safety, appropriate monitoring, and decisions guided by evidence.

For patients living with hidradenitis suppurativa, continued research offers the possibility of better treatment options and more sustained disease control. Although not every investigational therapy will demonstrate sufficient benefit, well-conducted studies provide evidence that can guide future research and treatment decisions.

As a dermatologist at Pantai Hospital Batu Pahat, I value the opportunity to connect clinical practice with research while continuing to learn from colleagues internationally.

Looking Ahead

The investigator meeting in Tokyo was a meaningful experience, combining scientific learning, international collaboration, and professional friendship.

I am honoured to serve as a Principal Investigator in this hidradenitis suppurativa study and look forward to contributing to research that may improve the care and quality of life of people living with this challenging condition.

My sincere thanks to the meeting organisers and fellow investigators, and to Professor Hayama for his generous hospitality during my visit to Japan.
