I had the pleasure of speaking to fellow general practitioners at Fika Café, Batu Pahat, during an educational session sponsored by Hyphens Pharma.

My talk, “Beyond the Surface: Balancing Sebum and Reinforcing the Skin Barrier,” explored how sebum production contributes to acne and why skin barrier care deserves attention throughout treatment. The discussion also covered Winlevi (clascoterone 1% cream) and its role as a topical anti-androgen treatment.

Understanding Sebum’s Role in Acne

Acne develops through several interacting processes, including excess sebum production, blocked follicles, Cutibacterium acnes and inflammation. Understanding these mechanisms helps guide treatment according to each patient’s presentation and needs.

One focus of the session was the influence of androgens on sebaceous gland activity. This pathway provides the basis for topical anti-androgen treatment, adding another approach to the management of acne.

For patients who would like to understand more about why acne develops, available treatments and expected improvement, I have discussed these topics in my related article: Acne (Pimples): Causes, Treatment Options, Why It Keeps Coming Back and What to Expect.

Winlevi: A Different Approach to Topical Treatment

Winlevi (clascoterone 1% cream) is a topical androgen receptor inhibitor used to treat acne vulgaris. It targets androgen receptors in the skin, offering a different mechanism of action from established treatments such as topical retinoids and benzoyl peroxide.

During the session, we discussed where this option may fit within an individualised treatment plan, taking into account acne severity, skin tolerability and other treatments being used.

Introducing a treatment also requires clear patient counselling. Explaining how to use the medication, setting realistic expectations and reviewing progress are essential parts of ongoing care.

Supporting the Skin Barrier During Acne Treatment

Treatment tolerability was another central theme. Dryness, redness, peeling and irritation can make it difficult for patients to follow an acne treatment regimen consistently.

Gentle cleansing, suitable moisturisers and sun protection can help support the skin during treatment. These measures deserve a place in the treatment plan from the outset, with adjustments when irritation develops.

In everyday practice, effective acne care involves both controlling the disease and helping patients maintain a routine they can follow comfortably over time.

Sharing Practical Insights with GP Colleagues

Acne is frequently encountered in primary care, and this session offered an opportunity to discuss both evolving treatment options and familiar challenges in daily practice. The connection between sebum regulation, treatment tolerability and skin barrier care provided a useful focus for the discussion.

My thanks to Hyphens Pharma for sponsoring the session and to the GP colleagues who joined us at Fika Café. I valued the opportunity to share dermatology insights with fellow doctors in Batu Pahat, particularly on how to make acne treatment effective and manageable for patients.
